Please bring with you:
- Your Medical Records: You can get your medical records from your primary physician or hospital. Contact your hospital to find out what the procedure is for obtaining records – this may take a few weeks so please plan well ahead. It will involve you going to the hospital to sign the release forms for these records.
- Relevant family history: Gather information of any diseases and disorders that affect your family. The more you know about your family’s health, the better we can help you. Try to find out about your parents and siblings, and your grandparents, uncles, aunts, and cousins as well.
- Insurance papers and/or payment: Once you have made an appointment, you need to pre-arrange verification of payment. Have your insurance papers handy when you phone and be prepared to take notes of where you may need to go for additional information.
